New Patient Process

We want this to be the right fit for you and your family. It is important to review our website, our vaccine policy, our financial and billing policies, etc. to be sure you are on board with our office structure. We accept new patients under the age of 15yrs old.

To set up an appointment at our office as a new patient we will need the previous medical records. You can fax, email, mail or hand deliver these to us. But, this process could take some time so make sure to request these from your previous pediatrician ASAP.

Call us to schedule your child’s next well visit. We will require a new patient deposit to secure the appointment which will be refunded to you after you have your first new patient appt.

Welcome to Children First! You will be texted a link to complete some forms prior to your first visit. If you need a sick appt or a more urgent appt before your well visit you can either call and schedule or follow the instructions online for virtual visits. We look forward to meeting you!
